Where's the line?

It's part of a Dominants role to ensure their submissive is taking care of themselves. I recently realised how easy it is to let that slip, especially if the real situation is being masked. But isn't it also the responsibility of the submissive to take care of their own health and well being? Of course they may need support to do so, but to serve their Dominant fully they need to be the best they can be. Of course ill health affects us all from time to time, and some may have a physical disability, but with good communication and planning they can be worked around. So where is the line? And if sub refuses to address the issues and seek professonal help where needed, what then?
